Liability Waiver

I, the parent/guardian, hereby agree and declare that I have carefully read and understand the scope of the summer camp activities and I consent to the participation of the above-named child to these activities.

I release and hold harmless the Sugar Land Art Center, its employees, students and agents from any and all liability for any loss, damage, injury or expense that I or my child may suffer as a result of my participation in the Sugar Land Art Center Camps including but not limited to, sickness, COVID-19, accidents, acts of God, war, civil unrest, terrorism, transportation, scheduling, government restrictions or regulations and any and all expenses I may incur while participating in the event. I understand this agreement cannot be modified except in writing by the art center and that no oral modification or interpretation shall be valid.

I have read the Liability Waiver above carefully and by checking this box, I agree to this information and acknowledge my responsibilities and the effect of this liability waiver. I give permission to take pictures of my child in classes for publicity in newspapers, websites, or brochures in an effort to promote Sugar Land Art Center for future years. I allow Sugar Land Art Center supervising staff to notify me of other upcoming learning opportunities.
